Terms and Conditions for First Team Trials with Fursan Hispania FC

I acknowledge and accept full responsibility for my participation in the Fursan Hispania FC program at the Academy trials at GEMS WINCHESTER SCHOOL Dubai. I further acknowledge that there is a potential to be injured in athletic events associated with the academy trials.

I understand that I am attending the soccer/football program and using the facilities at my own risk. Fursan Hispania FC, GEMS WINCHESTER Schools Dubai and all associated facilities, associated sponsors, venues and its/ their owners, employees and agents, shall not be liable for any damages whatsoever arising from any personal injury or property loss sustained by me as participant and my family/friends/colleagues/supporters in regard to any programs on the premises.

I acknowledge that I am aware of the risks inherent in my participation in soccer/football (both practice and competition); that soccer/football is a physical sport which can require considerable running, starting, stopping and physical exertion; and could potentially lead to or may cause serious or grievous injuries, including bodily injury and/or death. Participants assume full responsibility for all injuries and damages which may occur in or about any programs on the premises and I and my supporters hereby fully and forever release, discharge and hold harmless Fursan Hispania FC, Gems Winchester School Dubai and all associated facilities, associated sponsors, venues and its/their owners, employees and agents from any and all claims, demands, damages, rights of action, present or future resulting from or arising out of any person's participation in any programs or use of its facilities as mentioned above.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that health authorities still recommend practicing social distancing.

I further acknowledge that the Club has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that the Club can not guarantee that I will not become infected with the Coronavirus/Covid-19.

I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others.

I voluntarily seek services provided by the Club and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all procedures to reduce the spread while attending the appointment.

 

I attest that I:

• am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

• have not traveled internationally within the last 14 days. 

• have not knowingly been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. 

• have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by health authorities.

• am following all recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

 

I hereby release and agree to hold the Club harmless from and waive on behalf of myself, my child, my heirs, and any personal representatives any and all causes of action, claims, demands, images, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act, or that may otherwise arise any way in connection with any services received from the Club. I understand that this release discharges the Club from any liability or claim that I, my child, my heirs, or any personal representatives may have against the Club with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from the Club.

 

In addition, I agree that I will follow the rules of play and conduct set by Fursan Hispania FC and I understand that failure to do so may result in suspension from participation. Also, I waive all rights to any photos taken for the use of Fursan Hispania FC publication.

 

I confirm that I am over the age of 18 years old.
If possible, please bring two shirts of your own: a black one and a white one in order to be classified in the different trial teams.​
